Booking an Appointment
When booking for your appointment, let our staff know a short reason why you need an appointment and how quickly.
If you need to have vaccines given during your appointment, please let us know in advance to make sure we have enough supply for your appointment.
Before your Appointment
Review your Medication
Review all your medications (this include: puffers, prescribe creams) to see if you need refills. We do not fax refills to your pharmacy.
Make a List
Make a list of what you would like to discuss (this way you won't forget anything). If you have too many issues, we may have to defer some of them for another appointment.
Forms to Fill Out
If you need any forms to fill out, please make sure that you fill out your part before coming to the office. Make sure to bring the entire form with you. Do not start to fill out the provider part. We understand that you may want to save us some time, but filling out our part will only result in us refusing to fill it out and asking you to get another copy. If you want to help us, you can write some important information on a blank page to give to us instead. Some forms will not be ready the same day as they take longer to fill out. Our staff will contact you when they are ready. There is a charge for the majority of forms. Let our staff know that you have a form to fill out and they will let you know how much it is.
