Before You Visit | Full Circle Health Care

Please arrive 15 minutes before your appointment to complete the paperwork that is required.

For your first visit, plan more time as it will take longer to go over your medical history.

You will need to bring:

Your Medicare Card

We need an active card from your current residency province.

Medical Issues

Please prepare a list of your past and current medical issues, and surgeries.

Family Medical Issues

A list of your family medical issues for your first degree relatives (your parents, brothers, sisters, or children).

Current Medication

All your current medications (prescribe medications, over-the-counter medications, puffers, prescribe creams, vitamins, supplements or any natural products that you take). It is best to bring your bottles with you or a current medication list from the pharmacy.

Vaccination Records

Please bring your vaccination record. You can contact public health to get a full list of your vaccinations. This will help us to know if you are up-to-date.

Screening Tests

Please try to remember when your last screening test was for: Mammogram, Bone Density, Colonoscopy, and Screening Blood Work.
